Why should I sign up?
Signing up for Chronic Care Management is beneficial because it can help keep you out of the emergency room. Rather than facing the physical and financial strain of an ER visit, your insurance prefers to support preventative measures like CCM. By participating, you enable your Care Team and Practice to receive reimbursement for the care they're already providing, while also ensuring you utilize your insurance benefits effectively.
How much does this cost?
The cost varies depending on your insurance coverage. We bill your insurance each month after providing a minimum of 20 minutes of non-face-to-face services. You or your supplementary insurer may be responsible for any deductibles or copays. However, those with supplemental insurance or Medicare typically won't have a copay. If you're concerned about potential copays, consider signing up for our discount program, which offers patients the opportunity to receive 50%-100% off ALL copays.
What should I know before signing up?
For many patients, this program works exceptionally well, offering personalized care and consistent check-ins with healthcare providers. However, we understand that not all programs suit everyone’s needs. If, for any reason, you find that the CCM program doesn’t fit well with your lifestyle or healthcare goals, you have the option to discontinue participation at any time.
What happens if I discontinue the program?
Should you decide to discontinue, the provider will continue offering CCM services until the end of the month, and billing to your insurance(s) will continue for services rendered during that period. After the end of the month, CCM services will be discontinued, and no further billing will occur.
